Procedure of a consultation
The basic procedure for a medical consultation is as follows.

First visit
1. Reception of the first visit

Our center accepts patients by referral appointment only.
When visiting us, please have a letter of referral from your primary care physician ready. We would ask you about your medications you are taking.
2. Outpatient reception

Please show your patient registration card and reception ticket to an outpatient reception desk.
3. Consultation and examination

Please pick up the day's medical consultation ticket, examination reservation slip, and out-of-hospital prescription.
4. Outpatient reception

When your consultation or examination is finished, please return your consultation ticket, reception slip, etc. the outpatient (examination) reception desk.
For some departments, you may be asked to go to the next appointment reception desk.

5. Payment

Your reception number will be displayed on the monitor when your payment is ready. Please make your payment at the checkout desk (counter No. 4) or the automatic payment machine. You can also pay with a credit card.
The out-of-hospital prescription will be stamped with an " paid " stamp.

Follow-up visit
1. Reception machine for follow-up visits

Please check in at the Follow-up check-in machine on the day of your appointment.
If it has been 3 months since your last visit, you will not be able to use the Follow-up check-in machine.

Medical Questionnaire
For first-time visitors, please fill out a medical questionnaire before your medical consultation.
To save time, we recommend that you fill out the form in advance and bring it with you.
Please note that depending on the department you are visiting, you may be asked to fill out another medical questionnaire specific to that department. Please be careful to avoid losing the form, as it is your personal information.

Note: * indicates special outpatient departments for inpatients.
- Please bring the out-of-hospital prescription you received to the insurance dispensing pharmacy within 4 days and receive your medication upon payment.
In cooperation with the Tochigi Prefectural Medical Association, Utsunomiya City Medical Association, and other medical associations, we are able to follow up on your condition. - After finishing specialized treatment at the center, we ask for continuing medical care along with a report to the physician who referred you to us.
If a patient or family member requests, we may ask an appropriate doctor to provide medical care with a letter of referral (medical information form) following communication by phone or other means. - Explanation of examination may be given at Reception B.

If you wish to obtain a medical certificate or certificate, please ask at the Central Reception Desk. (If you have the designated form, please bring it with you.)
