Admission Procedures

Pre-admission contact

Once the admission date is confirmed, our admission staff will contact you by phone. We will contact you on the day before your admission. Please note that even if you have reserved your admission date, the date may be changed depending on the availability of hospital rooms.

Also, please understand that you may not be able to be admitted to the room of your choice depending on the availability of hospital rooms.

Admission reception hours

On the day of admission, please come to the admission desk (1st floor of the Main Building) between 9:30 am and 10:30 am.

However, if the admission time is specified separately, please come at the designated time. For those who are admitted in the morning, lunch will be prepared.

Items to submit at the time of admission

Please submit the following to the admission reception desk.

  • Patient registration card
  • Pledge
  • Health insurance card (including eligibility certificate for ceiling amount application and certificate of public expense receipt, etc. if applicable)
  • Questionnaire for admission (related to influenza and infectious gastroenteritis)

Cart

If you have a lot of luggage when you are admitted or discharged, a cart is available at the outpatient department. Please use it.
